Case Manager needed for large established Detroit area not for profit agency.Seeks professional with genuine interest in services to the elderly and disabled community. Working in a team environment you will conduct assessments, develop client care plans, monitor and report on care provided. If you are min licensed RN, and have background in a Home Health Care Setting or as a Care Manager working with a geriatric population, and supervising 12-14 case workers, we are looking for you.
